Thank you so much for making an in-kind donation of products or services to Lee Carlson Center. A donation form must be completed for every in-kind gift (tangible or intangible). This form is used to provide information to accurately describe the item or service and to prepare receipts for tax purposes.

To ensure proper acknowledgement, it is very important that the address, zip code and phone number are complete and accurate.

Bridgeview CSP is one of the many mental health programs offered by Lee Carlson Center. Lee Carlson Center is a 501 ©(3) non-profit organization. Your donation is tax-deductible as allowed by law. Please check with your tax expert for more details.

If purchasing items at retail, please include a copy of the receipt when you give us the donation.

Tax Information: If your in-kind donation is valued over $500, you must fill out IRS Form 8283. If the item is valued at over $5,000, you need to provide an appraisal. Should you have any questions, please refer to IRS Publications 526 “Charitable Contributions” and/or consult your tax advisor. Please note that contributions of services are not deductible as charitable contributions.
